Instructions for Forms

A) Saving a Heart Smart Foods Form to Complete Later

1) If you need to save the information
you have already entered on your form in order to complete and submit later, scroll to the end of the form and click on the white Save & continue later button to the left of the green Submit button:

2) Saving options:

     a) Google account
     b) Facebook account
     c) Create an account
     d) Skip Create an Account (small blue text link at bottom of window)

If you select option a, b or c, it will create a free Jotform account for you and the platform will send you a session email with a link to your form in progress.

3) If you select option d, the next window will appear:


The email address (e.g. johnsmith@gmail.com) that you filled out in the email address field of the form will automatically appear.

4) Click on the blue Send Link button to the right of the email address field and the following window will appear:


In addition,  a Session Email will be sent to your email inbox – we recommend checking your email to make sure you received it before closing the above window in your browser. If you cannot find the email in your inbox, click on the blue Get Sharable Link button or white Resend Email button.

5) When you open the email (from sender: JotForm; subject line: Continue filling out this form later), it will appear in the following format:


If you click the green Complete Submission button in the email, your partially completed form will open and you can continue filling it in.

6) If you copy and paste the link to your form (below the submission button) into your browser, you will be prompted with the following window:


If you click the Discard & Start Over blue button on the left, a blank, new Heart Smart Foods form will open. If your old partially filled session is still saved, you can always go to it if you have saved the link provided in your email. Otherwise, it is considered lost as sessions will not be kept due to security/privacy reasons.
If you click the Continue with Draft green button on the right, your previous, partially completed form filling session will automatically load on your browser for you to continue.

7) If you need to save the partially-filled form in multiple sessions, you can repeat from step 1 above. If you save multiple sessions, make sure to open the most recent email from Jotform in your inbox in order to access the most complete form to-date.

8) When your form is finally complete, click on the green Submit button at the end of the form:

B) Signing the Online Heart Smart Foods Form

Option 1 – Drawing Your Signature

This option will only work if the device you are using has a touch screen, like a smart phone, tablet or some kinds of laptops. If you do not have a touchscreen, please see Option 2.

The signature section of the form looks like the image below. Click in the signature field….


The pop-up window that opens looks like the image below. Click on the Draw button….


Use your finger or stylus to write your signature on your device screen as in the example below. You can clear any attempt and start again (it does get better with practice!) and when you are satisfied, click the blue Use button on the bottom right:


The pop-up will go away and you will see the signature you wrote inside the signature field:


Option 2 – Typing your signature

If you do not have a touch screen or if you would prefer to type your signature, click on the Type button as in the image below:


Position your cursor where it says Sign Here and use your keyboard to write your name as in the example below. There are different fonts to choose from if you click Change Style button. When you are satisfied, click the blue Use button on the bottom right:


The pop-up will go away and you will see the signature you typed inside the signature field:
